Media Permission Acceptance - you grant to Northbridge Dance Studios (“NDS”) , the right to take photographs/videos of your child whilst participating in dance classes/activities run by NDS. You acknowledge that NDS will use such images/videos for promotional purposes across various media including NDS' website, NDS' Facebook, NDS' Instagram, newsletters, print collateral, etc.
You hereby permit NDS to use images/videos in a lawful manner for the purposes described above. You understand that your consent can be withdrawn at anytime upon written notice and that students’ names and private information will not be disclosed.
You can either agree or disagree to the Media Permission as per the tick box below. If you agree to the Media Permission, then you grant to NDS the rights as stated above.

Some physical contact may be necessary by the instructor to demonstrate exercises or techniques during classes/rehearsals, or if a member of staff needs to administer first aid.
Participation in any Northbridge Dance Studios (“NDS”) activity and use of any recreational facilities involves a risk of accidental injury despite all safety precautions.
I have read the NDS Dance School Guidelines and have been informed of the activities conducted by the NDS and I/we, as an individual or as a parent or guardian of the participants named herein, assume all risks and hazards incidental to the activities, and release from responsibility and agree to indemnify and hold harmless the NDS, its principal, independent contractors, volunteers and all employees for any illness or injury to me or my children or family members occurring during his/her/our participation in any activity or use of any recreational facility at or conducted by the NDS.
